• Opened in 2001· Provides transition housing and supports to non-palliative clients wishing to successfully establish themselves in independent living environments
• Residents are provided with their own bedroom and share and maintain the other areas of the home.

Project 2011

• Opened in 2008 - Provides longer term housing and supports for individuals who have demonstrated an ability to live more independently in a group home setting.
• Residents are supported by a resident mentor living in the home.
