Case Management Beswick House (Long Term or Palliative Care) Scott House (Transitional Care) Education
Programs & Services

The SHARP Foundation is committed to providing a continuum of care including housing, healthcare and support to those individuals infected or affected by HIV/AIDS.  To create healthy environments, and promote health for persons living with HIV/AIDS, the Foundation will develop and enhance services and programs for individuals requiring palliative, long term, transitional and independent living support.  Residents will participate in the assessment of their care plans, education and personal development needs as an integral part of the programs and services provided by The SHARP Foundation.

The SHARP Foundation would like to inform our partners of some changes within our organization.  In May, 2006 The SHARP Foundation launched a two-year case management pilot project. The project was successful and is seen as a very valuable part of our programs.

In response to the end of the pilot phase of the project, The SHARP Foundation has implemented a case management team that will work together to address our client’s needs.

 This team is comprised of:

Janice Taylor – Senior Caregiver                                     403.235.3447

Nicole Dowling – Case Management Assistant                    403.248.1941

Heather Johnson - Recreation Therapist                           403.272.2912

Jaclyn Height – Executive Assistant                                 403.272.2912
